Kroger Community Rewards – Members Must Re-Enroll -Thank you to everyone who is participating in the Kroger Community Rewards program. In our first year of participation, the Fowler Community PTA has earned just over $900. Kroger gives non-profit organizations a percentage of your purchases back each quarter. However, it is important to know that each spring (April or after) members of the Kroger Community Rewards program must re-enroll by visiting www.krogercommunityrewards.com, click on “Michigan” and click on “enroll”. All participates must have a valid on-line account at Kroger.com and must sign up online to participate. Organization members can no longer scan their group’s barcode at the service desk. Thank you to those who have taken to time to link their Kroger cards to the Fowler PTA and if you have not already re-enrolled, please do so soon. We appreciated every dime!
Middle School Sports Physicals for 2010-11 - School sports physicals are required for students who participate in 7-12th grade school run sports. The Fowler Clinic will be providing free athletic physicals for the 2009-10 school year in the months of June and July to current Fowler Clinic patients only. Current Fowler Clinic patients need to make their own appointment during the months of June and July to receive their free athletic physical. Athletes who are NOT current Fowler Clinic patients must make arrangements with their regular doctor’s office to receive an athletic physical. All athletes must pick up and fill out an MHSAA athletic physical card from the school office and bring it with them to their athletic physical. Please make sure that both sides of the card are filled out completely and that both parent and student have signed the card. After the examination, the doctor must sign the physical card and return it to you. Students must then give the completed card back to the school. All athletes must have a completed MHSAA athletic physical card on file before their first practice or they will not be allowed to participate until they obtain one. The following instructions are a MUST:
1. The MHSAA athletic physical examination card MUST be obtained from the Waldron office and filled out completely before the physical. Take the completed card with you to the physical.
2. The athlete, parent, and examiner must all sign the completed card to make it valid.
3. On the bottom portion of the physical card there are two sections, one labeled Medical TreatmentConsent and one labeled Emergency Information. Both sections must be completed as this information is carried in the team’s first aid kit in case of medical emergencies.
4. If you have glasses/contacts, please wear them when you get your physical.
5. When the athletic physical is complete, the physical card must be returned to school
6. MHSAA Athletic physical cards may be obtained in the Waldron Office.
Any questions please contact Mike Spicer at 593-2160.
