We are currently utilizing an online enrollment process. You may be requested to download, fill out and return additional documentation other than the online form. copy of the SLD guidelines.
Please have the following items ready to upload before starting the enrollment form:
-Student’s birth certificate
-Up to date Immunization record OR a signed waiver from the health department
-Proof of residency (this can be any piece of mail or bill showing parent’s name and physical address)
Additional items to supply if applicable:
-Request for Records form (if you were previously enrolled elsewhere)
-School of choice application and release form
-IEP or 504 plan
-Guardianship documentation
-Foster care documentation
-Medical care plan
